CU 45. Pedicled skin flap onlay urethroplasty in treatment of complex male posterior urethral strictures: Report of 8 cases

Objective: the urological outcomes of pedicled flap onlay urethroplasty were retrospectively reviewed to study the general median-term effectiveness in the repair of male complex posterior urethral strictures.

Methods: Eligible male patients (n=8) who were previously diagnosed with complex PUS and urologically treated were hospitalized for receiving posterior onlay urethroplasty due to complex posterior urethral strictures. The pedicled scrotal septal skin flap was harvested and anastomosed with the urethral stumps through an inverted U-shape perineal incision and by using a modified Kirschner wire with 3-0 absorbable sutures. Antegrade urethrography and maximum urinary flow rate by uroflowmetry were examined postoperatively. The overall effectiveness was evaluated at 12 mo following the removal of urethral drain on the basis of lower urinary tract symptom, urethrography, the frequency of urethral dilation or DVIU, and maximum urinary flow rate.

Results: Out of the eight patients, six (75%) healed, and the other two patients with membranous urethral strictures had the recurrent strictures, requiring further urological interventions.

Conclusions: The pedicled flap onlay urethroplasty was effective for the repair of complex male PUS at a relatively lower recurrent frequency in the median term.
