CU 34. Prevalence of baseline chronic kidney disease in upper urinary tract urothelial carcinoma patients

Purpose: To evaluate the prevalence of baseline chronic kidney disease (CKD) in upper urinary tract urothelial carcinoma (UTUC) patients attending to Peking University First Hospital Urology Department from 2002 to 2010. To explore how much CKD patients had been ignored using the sCr as the only criteria to judge renal function in UTUC population.

Materials and Methods: Using the database of our hospital, we searched out and finally identified definitely pathologically diagnosed UTUC patients from 2002-2010. eGFR (estimated glomerular filtration rate) was calculated by Modification of Diet in Renal Disease (MDRD) formulas modified based on Chinese population. CKD stages were defined by the definitions of American National Kidney Foundation.

Results: 723 of 828 patients had appropriate record to calculate the Egfr (mL/min). Approximate 60% of all patients had CKD III+: 315 patients (44.57%) had CKD III, 55 patients (7.61%) had CKD IV, and 60 patients (8.30%) had CKD V. 492 of 723 patients (68.05%) had a normal sCr ≤1.4 mg/dL, in which, 199 patients (40.45%) had CKD stage III+. In the >70 years cohort, the same two ratios are 64.04% (180/281) and 54.44% (98/180).

Conclusions: More than half of all the UTUC patients have CKD stage III or higher, in the >70 years group, the ratio is greater (P<0.01). Use sCr to evaluate the renal function will ignore large proportion of patients suffering from CKD stage III+, especially in the over 70 years group (P<0.01). Compare with the western population, in our center, the prevalence of UTUC is lower in Chinese men compared with Chinese women (P<0.01).
