TCAS 09. New insight into chronic prostatitis

Both doctors and patients have to be faced with many difficulties in diagnosis and treatment of chronic prostatitis. Chronic prostatitis is an inflammatory reaction of prostate caused by pathogen infection such as microorganism or stimulation of some noninfectious factors, and the clinical manifestation including discomfort and pain of prostate region, paruria, and abnormal urethral discharge caused by the infection, and it is a common and very confused disease. The disease is currently recognized as a comprehensive disease or syndrome with its own unique form, which have its own special etiology, clinical features and outcomes, rather than a separate disease. Prostatitis is generally not life-threatening, but can seriously affect the overall quality of life, most patients were not satisfied with treatment, and many doctors felt difficult and experienced significant setbacks and disappointments during the treatment of prostatitis, and generally lacked self-confidence and the ability to make accurate diagnosis when facing the disease, which ultimately resulted in the appropriate treatment.

Therefore, changes is needed in many aspects concerning chronic prostatitis, including Definition and etiology, Epidemiology, Diagnosis, Treatment, Prognosis, Prevention, and Others. In this lecture, I will mainly focus my topic on follows (individual opinions):
1. Traditional and modern definition of chronic prostatitis,
2. Incidence of chronic prostatitis,
3. Clinical symptoms: No apparent characteristics for the clinical symptoms, Application of NIH-CPSI, and More emphasis on Quality of Life,
4. Unique feature of diagnosis: defective diagnosis,
5. Select requisite items to detect,
6. Treatment protocol: according to the clinical types and Select Combined treatment,
7. Traditional and modern cure standard for chronic prostatitis,
8. Prognosis: natural process and prognosis of chronic prostatitits,
9. The latest investigatory results on the incidence of chronic prostatitis(PUMCH data),
10. Education of the patients: Self adjust for the unhealthy Psychological factors and life styles,
11. Education of the docrors: Pay more attention to the details,
12. Prevention: protective measure for common people.

Summary

Take chronic prostatitis as a symptoms, rather than a disease entity; gold standard for the diagnosis dose not existed; target for the treatment is to release symptoms and improve quality of life; Combined treatment can expect to get good results; antibiotics is not important; cure or radical treatment is not a question; time is very important in the restoration process.
